The application form should be obtained from the college and filled in by the candidate in his / her own handwriting in BLOCK LETTERS, with Blue / Black ink.

At the time of reporting for admission, the candidate is required to produce all the documents in original and two sets of photocopies. If the candidate is admitted, these documents will be retained by the College till he / she completes the course. If the candidate fails to produce any of the documents listed above, he / she will instantaneously forfeit her / his claim for a seat.

Students who have migrated from other University / Boards and who are eligible for admission according to these rules will have to produce a certificate of eligibility from Saurashtra University to which the college is affiliated. (All candidates, other than those passing H.S.C. Examination of Gujarat H.S.C.E. Board will have to produce this certificate)

If any candidate finds it impossible to be physically present for the admission session due to unavoidable circumstances, he / she may authorize any other responsible individual to represent him / her for the admission session. This representative must carry with him / her letter of authorization as well as all documents listed in this prospectus. If the candidate or his / her representative fails to report for the admission, his / her claim for admission to B. Physiotherapy course will be forfeited.

Application with all enclosures should be sent to the college, either personally or by registered post to reach on or before 30th May, of respective year.
